Service Lamp: In good condition clean outside & inside. Glass chimney was free from soot smoke or stains.
Oil: Specimen examined & found to be free from all objectionable matter.
Service Lamp Burner: It was free from all gummy or burnt oil & wick was well trimmed.
Service Lamp Burner Level: It was found to be level. Keeper understands how to center it.
Plate Glass: In good condition free from scratches stains or dirt.
Lens Cover: It was on. Clean, hung up in the lantern room when not in use.
Lantern Curtains: 2 sets. Clean & in good condition: put up & taken down at the proper time.
Carillon or Alarm Bell: There is no alarm bell.
Rod Lamp: In good condition & very clean.
Dwelling: Dwelling is in good condition.
Stairway and Landing: In good condition, clean & painted as directed.

Heater: Keeper understands use of the heater.
Ventilation: Keeper understands use of the ventilators & how to air the bldg.
Dome: They were painted according to instructions; & in good order. Lightning conductor was in good condition.
